Application Requirements (Please use your legal name only) 1. Submit completed application and $30.00 processing fee (payable by check, cash, money order, MasterCard or Visa). The processing fee is refundable only if the application is not accepted for admission or if the application is cancelled by the applicant before the fourth business day following acceptance to Madison Media Institute. 2. Include two (2) letter of recommendation. The letters should be from current or past employers, former instructors or industry professionals. Letters from family members will not be accepted. Each letter should state why that particular person thinks you would be successful in your chosen program or field of study. 3. Include a brief 200 to 500 word typed essay describing your career goals. The essay should contain relevant background experience that has assisted you in choosing a career path. Also, describe how you expect Madison Media Institute to assist you in accomplishing your goals. 4. Include an official copy of your high school, GED, or HSED transcripts showing your graduation date. Transcripts must display school or testing center's official stamp and signature.
